Structured Family Caregiving (SFC) is a Medicaid-funded program in Indiana that enables a family member or other trusted individual to provide daily, in-home care for a loved one who otherwise would need nursing-home-level support and receive a daily stipend and support services for doing so. Its part of the states Home and Community-Based Services (HCBS) Medicaid waiver programs such as the Aging & Disabled Waiver, PathWays Waiver, Health & Wellness Waiver, and TBI Waiver.

Through SFC:

  • A caregiver lives with the care recipient and provides daily support.
  • The caregiver receives training, coaching, and a daily stipend.
  • A registered nurse (RN) or care team oversees and supports the care plan.

The goal is to help individuals remain at home in a familiar environment rather than be institutionalized.

Who qualifies & Requirements

  1. Care Recipient (the person needing care)

    To be eligible for the SFC program in Indiana:

    • Must be Medicaid-eligible and enrolled in an HCBS waiver (e.g., Aged & Disabled, PathWays, Health & Wellness, or TBI Waiver).
    • Must require nursing-home-level care
    • Meaning significant help with daily living activities
    • Must live full-time with the caregiver (in the same home).

    These criteria ensure the caregivers presence and support are essential and continuous.

  2. Caregiver Requirements

    To be the Structured Family Caregiver:

    • 18 years or older.
    • Live with the care recipient (same residence).
    • Physically and mentally capable of providing necessary daily care.
    • Willing to undergo a criminal background check.
    • Must be available to provide ongoing assistance with activities such as eating, dressing, bathing, transferring, etc., as part of the care plan.
